Welcome! We are excited that you have joined the Midlands Dental family. We want to make you as comfortable as possible before you even walk through our doors. Below you will find a list of things to bring to your first appointment, as well as an idea of what to expect during the appointment. Please do not hesitate to give us a call if you have any questions before you come in.

What to Bring to Your First Appointment

 
  • High blood pressure, diabetes, rhuematic fever, artificial heart valves and joints.

  • Please bring your insurance card to your appointment. If you do not have a dental insurance card, we require the carrier name and phone number, subscriber number, employer name and group number.

 

All patients under the age of 18 years must be accompanied by a parent or guardian at the consultation visit.

What to expect at Your First Appointment

 
  • We will discuss your medical and dental history to determine whether any information is relevant.

  • Having problems with your teeth? Are there any changes you would like to make to your smile? We can discuss those issues today.

  • If you have previous X-rays from your former dentist, we will review these.

  • We will perform a full examination of your teeth, gums, soft tissue, and bone support.

  • We will perform the tests necessary to diagnose your condition, including x-rays and/or diagnostic models of your teeth if there are problems with your bite or joints.

  • If you are experiencing pain, we will work to correct your issues or refer you to a specialist.

  • If further treatment is required, we will create a customized treatment plan to fit your schedule and budget.

We work to keep our services as affordable as possible while still upholding the same standard of care you have come to know and expect from Midlands Dental. Our financial policy is here so we can continue to provide clear and reasonable expectations for our customers.

We request that that the full payment be made after each visit except where there is a prior arrangement. In case a treatment is going to cost more than $300, we offer a number of financial options.

The insurance amount you are eligible for is based on the plan that has been chosen by your employer. Though a number of dental services are covered by insurance, many of them aren't. Our office will file the insurance claim on your behalf and we do everything within our reach to ensure you get maximum insurance benefit.

If your insurance accepts assignment and will send their payment to our office, we will be able to file your claim; however, we request that you pay your estimated portion on the same day you receive services.

Regardless of which insurance benefits you are eligible for, it is your responsibility to clear the entire balance once you receive our office statement. In most instances, it takes insurance companies up to 30 days to clear your claim amount.

In case there is any unpaid balance, you will be charged interest on a monthly basis. If there is a delinquency on an account, it will then be turned over for collection. All the costs of collection, inclusive of the attorney’s fees (which will not exceed 15%), will be added to the account and the patient is required to pay them.
